Gwendol M. Forrest, 97, of Clearwater, Florida passed away May 26, 2018 in Orange Park, Florida surrounded by her loving family. She was born August 25, 1920. Crevasse's Simple Cremation of Jacksonville is honored to be serving the Forrest family.
